Main changes:
- adding an identifying code for the combination of pt cuts in the muon trigger
  Look-Up-Table (LUT)
- modify AliMUONTriggerIO (called by the SHUTTLE) to transfer this code to
  the ROOT LUT in the OCDB (use the title of one of the LUT histograms to add
  this new information)
- modified files: AliMUONTriggerIO.cxx, AliMUONTriggerLut.h ,.cxx
- use AliMUONTriggerLut::PrintLutCode() to print out the code value
- insure back compatibility for older LUT files without this information
